Think maybe the problem is more that they use consultancies because although I suspect that countrywide all executive levels know where their patch's weaknesses and strengths are , obviously no managements or MP's want to admit any of their hospitals may need to be changed in some way as it could mean job cuts etc. I expect your piper theory has some credence too derek .'Independent' Consultants can be ex NHS employees .

Its also the blame-shifting, nobody within the organisation is prepared to stick their neck out.
All public bodies are the same, they are told to take risks and think outside the box, someone comes up with an idea but usually its only partly implemented and doomed to failure and then the blame game starts.
The ones with the teflon shoulders are those that have mastered the art of coming up with loads of words but nothing really changes, just different words and different paperwork.

Thats it , isn't it Mr X at high level wants to impress so has to make SOME changes to get noticed -or appease the level above him . Most of these changes have been tried before, but staff and bosses leave, and its all forgotten by everyone but 'sticker' staff who try hard to get message across that X's plan can't work. Impossible task , so new plan is forced into place at the cost of millions , and as you say DD , its doomed to fail. Total waste of money, over and over.
Then theres situations in public life where the bosses demand you MAKE it work by hook or by crook , to make the duff ideas work, and Ive heard that you can 'bend' employment figures for eg. very nicely by dividing staff into different catagories so they dont count in certain circumstances . All slight of hand and divisive , and no REAL savings or advantage. We really need to put more money in , we dont spend nearly as much as other comparable Euro countries - but we need to target better too, and stop all the shenanigans about setting impossible results just that look good and mean nothing.
